Community Wellness Program Toolkit

MWP4H has supported the development of a Community Wellness Program (CWP) Toolkit. The purpose of the CWP Toolkit is to consolidate information about all of the Community Wellness Programs offered by public health entities across Region 1 of Western North Carolina into one accessible resource. The toolkit is meant to support the development of new Community Wellness Programs and provide additional resources for existing programs. The toolkit includes information about curriculum, evaluation, challenges, incentives, and marketing tools. To create a more robust toolkit, there is also a section that lists national and non-local Community Wellness Programs and a section of other national and non-local health programs that are not specifically community wellness focused, but may have aspects that would be valuable for implementation in a WNC program. As participating Region 1 sites add components to their programs, they can be updated within this Toolkit as well, ensuring partners have access to the most current information and examples available. 

 
Curriculum and Evaluation Measures: WNC Community Wellness Programs

This document contains information about the curriculum and evaluation measures of the community wellness programs across WNC. The curriculum outlines the basic purpose of the program, the length of the program, how it is set up, and its different components. The evaluation section outlines how the program measures changes in health and health behavior as a result from the program; for example, some programs will check blood pressure and glucose levels at the start of the program and again at the end of the program to see if it has improved.

Challenges: WNC Community Wellness Programs

This document has information about challenges created by different WNC community wellness programs. The participants of these wellness programs work to achieve goals set in the challenges in order to earn incentives or prizes. For example, a challenge may be that each participant walks 30 minutes every day for a month. At the end of the month, participants who have met the goal and completed the challenge earn a prize.

Incentives: WNC Community Wellness Programs

This document includes information about the different incentives that current WNC community wellness programs give to their participants. These programs have goals that their participants work to achieve; if they achieve the goals of the given challenge/program, they are given rewards or prizes of some kind. For example, if a program has a challenge where the participants walk 30 minutes a day for a month and the participant achieves this goal, they could receive an incentive of a gift card.

Marketing Tools: WNC Community Wellness Programs

This document includes information about the different marketing tools used by current WNC community wellness programs. A program cannot be effective or successful if no one knows about it! Therefore, it is very important to effectively spread the word about the program and reach target populations so they can be enrolled in the program. Advertising in the newspaper, on the radio, and through flyers are just a few examples of marketing tools described in this document.

Non-WNC Programs

In addition to local WNC programs, there are many additional programs in the state of North Carolina and nationally. This document has information about other programs that might have components that are useful for implementation in the development of new community wellness programs.
