Welcome to the NC Minority Diabetes Prevention Program in Swain County
Thanks for your interest in the North Carolina Minority Diabetes Prevention Program (MDPP). We are excited to help you register for the program and get connected with a dedicated Lifestyle Coach.
A Community-First Approach to Health Supported by MountainWise and the NC Department of Health and Human Services, we are proud to bring diabetes prevention to your area completely free of cost.
We understand how important community is here in Western North Carolina. Our goal is to provide a supportive space for our clients to learn, grow, and prosper. By the end of our year-long program, participants will have the knowledge, tools, and skills to reverse prediabetes and avoid a Type 2 diabetes diagnosis.
What to Expect The program consists of 26 classes over the course of one year. Facilitated by Certified Lifestyle Coaches, participants will learn how to:
-
Track physical activity and daily nutritional intake
-
Get active and build sustainable, healthy habits
